Refer a patient
Mercy Health is now accepting electronic referrals via HealthLink. We accept GP and specialist referrals for all specialist clinics. Referrals are triaged against clinical information and investigations using HealthPathways Melbourne and Statewide Specialist Clinic Referral Criteria, developed by the Department of Health.
Shared maternity care affiliates
To participate in shared maternity care as a health professional, GPs or community health centre midwives must be registered with Mercy Hospital for Women or Werribee Mercy Hospital as an credentialed antenatal care provider. Learn how to become a Shared Maternity Care Affiliate.
Public Fertility Care Service
Mercy Hospital for Women is proud to be part of Victoria’s world class Public Fertility Care Service provides access to comprehensive fertility treatment including genetic counselling, fertility preservation, and fertility assessment and treatment.
Accessing your patient's information
Freedom of Information (FOI) provides advice on information for the release of patient information for health services in accordance with the Health Records Act 2001 and the Health Services Act 1988.
My Health Record
Mercy Health advocates for better communications between healthcare providers. Mercy Health will upload inpatient electronic discharge summaries, pathology and radiology imaging results and specialist clinic letters to a patient’s My Health Record (where one exists).
Advance care planning with your patient
Advance care planning can help patients receive the treatment of their choice in the future. It also helps their family and doctors to understand what they want and what is important to them.
Statewide Referral Criteria
The Statewide Referral Criteria provide guidance to referring clinicians about which patients will be seen in adult public specialist clinics and what investigations are required to support triage. HealthPathways Melbourne provides clinicians with a single website to access clinical and referral pathways, and resources. Pathways are created by local GPs, working in partnership with specialists, nurses, allied health and other health professionals.
Mercy Health recommends clinicians to access HealthPathways Melbourne for guidance in assessing, managing and referring for patient conditions (login required).
Referrals to outpatient clinics at Mercy Health will be triaged against clinical information and investigations based on these guidelines.
Referrals that do not meet the required clinical information and investigations will not be accepted.
HealthPathways Melbourne is available for free to health professionals working in the North Western Melbourne and Eastern Melbourne Primary Health Network catchments.
